Your First Visit

During your first visit to receive physical therapy or occupational therapy at 3A’s Therapy Services in San Antonio, TX, it is a good idea to arrive at your appointment with your paperwork completed. You can download the forms on our website or check your email where the forms will be sent when you book your appointment. If not, come 20 minutes early and our friendly office staff will help you complete the paperwork.

Present your prescription order for physical therapy. You need to have a doctor’s referral or prescription to access our services.

We will copy your insurance card and you will then be taken to see the physical therapist.

During the initial evaluation, your medical history will be taken and your current problem discussed. We want to know not only the nature of your pain and discomfort, but also what it prohibits you from doing in your daily life. We will ask your injury or condition is impacting your daily activities and your goals with physical therapy.

Please bring with you a list of any prescribed and over-the counter medications that you take regularly.

After a thorough evaluation of your pain and injury, we will customize a physical therapy plan for you. It will not only discuss the therapy that you need, but the number of visits per week that is recommended.

You will be prescribed a series of therapeutic exercises to do at home between visits so that you can be a partner in your own care. You will also have a series of goals both short-term and long-term which will be created with your input.

Please were comfortable, loose-fitting clothing that allows you to move easily and that makes it easy for the physical therapist to easily access the injured area.


Insurance

At 3A’s Therapy Services we accept most major insurance providers including Blue Cross and Blue Shield of Texas, United Healthcare, Medicare and Medicaid as well as TriCare, Aetna, Humana and Cigna.

If you have specific questions about your insurance coverage, our office staff will be pleased to find the answer to your questions.

We also accept cash payment and credit and debit cards (a fee applies for credit and debit) and will file for reimbursement for you through your insurance company.

Please print and complete your intake and screening forms that will be sent to your e-mail after you schedule an appointment by email or phone with 3A’s Therapy Services in San Antonio. Bring them with you on your initial evaluation visit.

If you are unable to print your forms or do not have an email address, do not worry because our friendly office staff will have your forms ready to complete at the clinic.

If you will be completing your forms at the clinic, please kindly arrive 15-20 minutes before your scheduled appointment.

Your responsibility for payment at each visit will be based on your insurance benefits. Depending on your benefits, you may be responsible for a co-payment or deductible.

Our office staff will be happy to give you an estimate of your financial responsibility at your first appointment and provide you with treatment plan options.
